Perhaps it is not a sexual problem, concern, but a relationship/relating problem. Instead of looking at the sexual encounters look at the relating/relationships. Maybe the client has difficulty with relating to others and only knows how to relate sexually. Or, does not know how to get close to people intimately without being sexual. That changes the perspective.
