Actually, surgeons now ARE doing surgery via the internet.

Regarding projection/transference occuring via text relationships. Psychoanalytic theory (which created these concepts) easily could predict that these things would occur in the heightened ambiguity of text communication. One of the reasons why analysts sit behind their clients is to eliminate visual cues, which heightens the "blank screen" effect resulting in increased projection and transference. Not unlike the lack of visual cues (along with auditory cues) in text relationships.

Aside from theory, it's quite clear that projection and transference reactions happen often in online text communication, including online therapy.
