I work in a different context, the UK, but i would still ask myself what information do I need to record that would be in the client's best interests and what information is not entirely relevant. the client must have confidence in the overall setting that the therapits provides, including how and what information is stored. where a client has anxiety about it has been my expereince that this is related to an underlying problem and not just what information are you holding on me and can be thought about.
Much is to do with the setting of course but lengthy details of contacts are of not much use to anyone.
I argee that confidentiality is an impotant boundry but it can never be a completey sealed part of therapy, if a client makes such a demand, therapy is not possible.
