If your insurance was billed for sessions that did not occur, or that occurred out of the office, you should notify your insurance company. Managed Care Organizations have rules and do not hesitate to remove therapists from their panels for any claims of wrongdoing (there doesn't even have to be proof). They may well investigate and if a pattern of such abuse is found they might even prosecute.
Not all therapists behave this way. In fact, I don't know any who behave this way. It's unusual, odd, and most likely unethical.
