For a very informative and detailed text on "ego state" treatment protocols, I would suggest John and Helen Watkins' book on "Ego State" treatment. I would give you sound and detailed strategies for this (and many other) problem. If you are not familiar with this treatment, I suggest you seek supervision with an experienced peer during treatment. It could be very effective and/but powerful, and supervision could be valuable in dealing with powerful affective "ego states" of hurt and malevolence common with a person with this kind of experience/story.
