What is the quickest, most effect method to help a child with this disorder. In question is a 10-year-old boy, with generalized anxiety since September, 1997. No history of any crises or precipitating trauma. Similar, but shorter episode at age 5 when he was signed up for indoor soccer and quit after 2 games ...classic symptoms of stomache ache, insomnia, nightmares, crying at that time. During present episode, he attributes anxiety to "missing the summer" ... had a good summer, enjoyed playing on computer, playing with friends/brothers, staying up late, and first time family vacation to Florida. Started fretting about returning to school but this was considered the normal reaction of kids at the end of summer. Changes however; placed in French Immersion class where he did not know most of his classmates; new teachers; new curriculum (mostly in French); for the first time he was alone at his school because his older brother (who always accompanied him home on the bus, etc.) was now away at highschool; financial problems in family. I have tried reassurance, supportive psychotherapy, relaxation therapy, some behaviour therapy and cognitive therapy but perhaps too much for a 1o-year-old (although he is highly functioning for his age and very insightful). Also, he DOES seem to be improving; however as with separation anxiety and school phobia, there are relapses when the child avoids school; similarly, in November there was a 2 week teacher's strike so he was happy at home but regressed when classes started and had some tearful days . Again, experiencing this after returning to school after 2 week Christmas holidays - not as severe, however. Therefore he does seem to be improving but still exhibits symptoms of anxiety, expecially in the morning ... perspirating, racing heart, headaches, stomaches, tearfulness, shakiness - but htis seems to be improving. There is a strong family of anxiety in a mother and paternal grandfather. Any suggestions? Will this child be predisposed to adult anxiety disorders? We want to help stave that off if possible. Looking forward to any help in this case...
