From an ethical point of view, it would be necessary
to become a Professional or Registered art therapist
if you are considering art psychotherapeutic work
with clients who present with TBI, stroke, etc.
It may therefore be necessary to check these ethical issues
out with your Professional Speech and Language Pathology
Association in terms of associated with exploring
psychological issueswith your clients because it's a
question of professional competency. It would be similar
to practicing medical procedures without being a doctor.
    Good luck.
    Steve
