Question Do you use assessments?

I have recently begun marketing an assessment tool for use by mental health professionals and have been surprised at the number of psychotherapists who do not do much assessment with their clients. I am convinced that knowing your patient's “Psychological Vital Signs” is as important for counselors as physical vital signs are for M.D.s.

Do you use assessment tools? If not, why not?
