Very interesting book! I think he may be using "narcissism" in a very general sense - similar to how psychoanalysts talk about "narcissistic pathology" or even "borderline disorders" in that there is a significant disruption in one's sense of self (identity, self cohesion, self continuity) that results in pathological behavior. A "narcissistic personality disorder" (which the author at times seems to be emphasizing as his main concern) is really something much more specific. It's a quest for attention, admiration, and praise in order to bolster a false sense of self-importance and grandiosity.... We do see quite a bit of that online.
