diagnosing"brief psychotic disorder" requires one of1)delusions 2)hallucinations 3)disorganized speech 4)grossly disorganized or catatonic behavior.i fail to see the basis for this diagnosis which would seem to me to have much prognostic import as well as very relevant to choice of therapy.it would seem to have played a major role in the decision to hospitalize which i also dont understand. fleeting suicidal thoughts seem an insufficient basis.the diagnostic evaluation is sparse but all i hear so far is Adjustment reaction with mixed anxiety and depressed mood. given the importance of the girl friend in his reaction i would have brought her in as soon as possible(i usually ask patients to bring their significant other to the first evaluation) at any rate i think the clinical speculations far outrun the sparse data and think that a problem since it is so easy to find material justifying one's appercus.
