If you're talkign about a client who dissociates or has flashbacks, it's important to get familiar with what that person's triggers are. I find it helpful to address whatever seems to be bringing on the flashback. For example, one client of mine is triggered by footsteps in the hallway, and ours is a thin-walled building. When I noticed the 1,000-yard stare on her, I continued to talk about the footsteps and tie them into what was making them at the time, talking about the professionals who use the back door adjacent to my office, how it was lunchtime and everyone was headed out to get something at McDonald's. This both kept her tied to the present and helped train her so that outside the office she can eventually do this herself. Also, I keep a dish of peppermint candy on my table, and if a client dissociates, I give them a simple command to take one and put it in their mouth. THe strong sensation works wonders. Some of my clients now keep a few in their pockets at all times.
