    I think GHQ may be the same questionnaire or a different version from Rand known as the Psychological General Well Being Index (18 items), I think the author is Dupuy.

    The PGWB is has anxiety, depressed mood, positive well-being, self-control, general health and vitality subscales and is in the public domain. General health assesses the degree to which the pt is bothered by illness, is healthy enough to do things and is concerned or worried about their health.

    In any case, it is very easy to administer by self-report and in a large community based study of older overweight adults, we found it discriminated well between adults who were successful in the program and between those who were able to start exercising and those who weren't. It was sensitive to change over time and folks who changed behaviors had improved PGWB scores.
