John Ware (http://www.qualitymetrics.com) will give you a copy of the SF36 pretty inexpensively, like the price of xeroxing, and then you can copy it as many times as you want. The catch is that the scoring algorithms are proprietary. best wishes,
The other option is the Health Status Questionnaire HSQ which Ware actually developed while at RAND (there are some complicated politics here into which I will not go). Suffice it to say, they are equivalent in terms of what they measure, the SF36 has some fancier scoring, they both have good normative data for a variety of clinical groups. The HSQ has a nice 3 question depression screener, developed by Rost, Burns, and Smith (1992?) from the University of Arkansas. That may give the HSQ the edge in primary care, PCPs I have worked with really liked that feature.
BTW, the American Medical Group Association has been using the HSQ in their Outcomes Consortium for about the last 7 years. They have a huge amount of data on the HSQ. If you or your organization have any links to the AMGA, they are a terrific resource.
